Application for an ash or burial interment

Applications for an ash or burial interment can be submitted by an Executor, Power of Attorney, Next of Kin, or Funeral Director.

Select the link below to complete an interment booking online. A medical certificate of cause of death or a death certificate is required as part of the booking process. 

If you use our online booking form, you do not need to complete the PDF forms 'Authority to Open a Plot' or 'Application for an Adjacent Purchase' as they will be prefilled and sent to you as part of the booking process.
